Regis College Sports Medicine Forms
The Regis College Athletic Department seeks to provide the best possible care for our student-athletes. Any information gathered, on both student-athlete and/or guardian, is considered confidential medical information. This information is part of the student-athlete's medical record and will help expedite medical care in the case of an illness or injury that they may sustain through participation in athletics.
All student-athletes must be cleared through the Sports Medicine Clearance process yearly through the Regis College Healthy Roster on-line record keeping system. Student-athletes will also be baseline concussion screened before participating in organized physical athletic activity. Information about the concussion screening process will be given after completing the necessary Healthy Roster clearance.
The Sports Medicine Department requests that this information is returned by the following deadlines:
- Fall Athletes – prior to July 1st
- Winter and Spring – prior to August 1st
Delayed submission may affect participation status of student-athletes and may affect their invitation to pre-season or ability to participate in team workouts or non-traditional practices.

Pre-Participation Exams/Physicals
The NCAA requires student-athletes who are beginning their initial season of eligibility to undergo a medical examination or evaluation. All first-year, transfer, and student-athletes that have not previously participated must have the examination or evaluation administered within six months prior to participation in any practice, competition or out-of-season conditioning activities.
The Sports Medicine Department requires that all first-year, transfer, third year, fifth year, and any student-athletes who have not previously participated in Regis College Varsity Athletics undergo a physical examination prior to participation. It is strongly recommended that all student-athletes undergo a physical examination every year. To ensure that student-athletes are participating safely, the Sports Medicine staff may require updated physical examination documentation/clearance for any student-athlete who is being tracked for a long-term medical condition or who has had a change in their health status since they last provided physical exam clearance.

Sickle Cell Information
The NCAA requires that all NCAA student-athletes have knowledge of their sickle cell trait status and they must provide documented results of a Sickle Cell Solubility Test (SST) that clearly states that status.
All student-athletes participating in their first year must either:
1. Provide proof of prior sickle cell testing and have them faxed directly to Regis Sports Medicine (781-768-8329) or upload directly to Healthy Roster
2. Complete testing through Regis College Student Health Services (call 781-768-7290 to schedule an appointment) or with their individual physician for a fee.
3. Find a local Quest Diagnostic as they have a code for the NCAA sickle cell test (no referral needed) - https://sicklecelltesting.pwnhealth.com/order
These results must be submitted into Healthy Roster in order to be cleared for any organized physical activity with a Regis College team.

NCAA SICKLE CELL TRAIT FACT SHEET FOR STUDENT-ATHLETES
ADHD/ADD Medication
The NCAA requires any student-athlete who takes ADHD or ADD medication to report all pertinent information to the Regis Sports Medicine staff. Required information includes doctors' diagnostic evaluations (see link to sample below) and doctors' prescription of medication.
· NCAA Medical Exception Document (print and provide to physician)
A medical exception document or updated letter from a prescribing physician must be submitted yearly or if there have been any changes in the prescribed medication or dosage.

Medical Exception Information
Effective August 2009 the NCAA will institute stricter guidelines for eligibility under medical exceptions for banned substances, including stimulants used to treat ADHD/ADD, medications for male-pattern baldness, peptide hormones (hGH), and anabolic steroids (testosterone) or any other medications banned* by the NCAA. There are no medical exceptions for street drugs.
NCAA BANNED SUBSTANCE INFORMATION
The NCAA bans classes of drugs* because they can harm student-athletes and can create an unfair advantage in competition. Some legitimate medications contain NCAA banned substances, and student-athletes may need to use these medicines to support their academics and their general health. The NCAA has a procedure to review and approve legitimate use of medications that contain NCAA banned substances through a medical exceptions procedure. The benefit of a medical exception procedure is that in most cases the student-athlete’s eligibility remains intact during the process. Any student-athlete who tests positive from the effective date will need to comply with this stricter application, even if that student-athlete had received an exception for the use of stimulant medication prior to August 2009.
Concussion Screening
All student-athletes perform baseline concussion screening prior to athletic participation. This process will take place after completion of the Healthy Roster Sports Medicine clearance process utilizing Impact computer testing and SWAY balance screening.
Regis College Requirements
Immunization and health records required by the university MUST be uploaded to the Regis College Center for Health and Wellness system. This system is separate from the HealthyRoster system used in athletics but some similar information may be required. Unmet university requirements will prevent any ability to move onto campus and participate in athletics.

All student-athletes must maintain personal health insurance and MUST enroll / waive the Regis Student Health Insurance Plan to verify that they have active coverage. The enrollment/wavier period will open on or around July 15th. An email with information about this process should be received directly from Gallagher Student Health & Special Risk and you may also receive a reminder from your coach or from a Sports Medicine staff member. No student-athlete will be allowed to participate in athletics without verifiable health insurance coverage.
