REQUIRED MEDICAL INFORMATION
The Regis College Athletic Department seeks to provide the best possible care for our student-athletes. Any information gathered, on both student athlete and guardian, is considered confidential. This information is part of the student-athlete's medical record, and will help expedite medical care in the case of an illness or injury. Please be aware of the following NCAA requirements.
Student-Athletes must have a complete medical record on file with Health Services as required by MA law. Part of this medical record includes a physical exam.
The NCAA requires student-athletes who are beginning their initial season of eligibility to undergo a medical examination or evaluation administered or supervised by a physician (e.g., family physician, team physician). The examination or evaluation must be administered within six months prior to any practice, competition or out-of-season conditioning activities. Please use the physical form provided by Regis College Heatlh Services
THIS PHYSICAL MUST DOCUMENT IN WRITING THAT YOU ARE CLEARED FOR PARTICIPATION IN INTERCOLLEGIATE SPORTS
Fall Athletes - Physical completed after 2/20/12
Winter Athletes - Physical completed after 4/12/12
Spring Athletes - Physical completed after 7/20/12
***A copy of your physical must be submitted to both the Sports Medicine Department and Health Services. You must mail a seperate copy to each department***

The NCAA is recommending that all NCAA student-athletes have knowledge of their sickle cell trait status. New requirements have been added to the pre-participation physical in which student-atheltes must either.
1. Provide proof of prior sickle cell testing and send with their physical forms
2 . Complete testing through Regis College Student Health Services
ALL FORMS must be completed and returned to the Sports Medicine Staff before participation. Incomplete forms WILL NOT be accepted. FALL ATHLETES MUST SUBMIT ALL FORMS BY AUGUST 13th, 2012. Students who do not have a completed medical file will not be invited to preseason.
